The original header is vigorously crossed out in pencil, with scoring notes. 100 yes or no questions, several with specific application to workers, including seeking out a gas leak (not with a match!), fashioning make-do gas masks, transporting injured men uphill, etc. Test taker Carrie Gehry was marked down for 11 incorrect answers--though their write-in answer to whether to treat sunburn the same as a first degree burn ("Depends on degree of burn"), should count as true.
