"The Mutual Life Insurance Company of New York printed "Plain Directions for Accidents..." and distributed it to its policy holders (with "Keep this where you can readily find it" appended to the title). "The two portions usually appear as separate pamphlets, but for convenience of preservation this limited edition has been prepared."
